Some people feel that getting injured while working can be a burdensome when filling out the paperwork and contacting your employer. RULE #1 - It is extremely important that you contact your employer when you have been injured while working. Your employer must provide you with a DWC-1 claim form within 1 day of knowing that you were injured while working.

** If your employer fails to provide you with this form, you should immediately contact the Workers Compensation claims administrator to obtain the information. The claims administrator is the person who is handling your employer’s Workers Compensation claims. Their contact information should be posted at your place of work in the area in which other employment posters and information is posted. You can also obtain the DWC-1 form at the Division of Workers’ Compensation website: www.dwc.ca.gov, then go to the navigation bar and find “I want to” and then click on “find a form” which then proceeds to the “DWC-1 form”.
